Indications/Uses
Treatment of schizophrenia, acute manic & mixed episodes, maintenance treatment of bipolar I disorder, adjunctive treatment of major depressive disorder (MDD), irritability associated w/ autistic disorder, agitation associated w/ schizophrenia or bipolar mania & Tourette's disorder.
Dosage/Direction for Use
Schizophrenia Initial dose 10-15 mg/day. Adjust dose gradually, increases should generally not be made before 2 wk. Recommended dose 10-15 mg/day. Max dose: 30 mg/day. 13-17 yr Initial dose 2 mg/day, titrate to 5 mg/day after 2 days & to the target dose of 10 mg/day after an additional 2 days, subsequent dose increases should be administered in 5 mg increments. Recommended dose: 10 mg/day. Max dose: 30 mg/day. Bipolar I disorder, manic or mixed Initial dose monotherapy: 15 mg/day, adjunct: 10-15 mg/day. Recommended dose: 15 mg/day. Max dose: 30 mg. 10-17 yr Initial dose 2 mg/day, titrate to 5 mg/day after 2 days & to the target dose of 10 mg/day after an additional 2 days, subsequent dose increases should be administered in 5 mg increments. Recommended dose: 10 mg/day. Max dose: 30 mg/day. Adjunct to antidepressants in MDD Initial dose 2-5 mg/day. Dose adjustments up to 5 mg/day should occur gradually, at intervals of no less than 1 wk. Recommended dose: 5-10 mg/day. Max dose: 15 mg/day. Irritability associated w/ autistic disorder 6-17 yr Initial dose 2 mg/day. Dose adjustments up to 5 mg/day should occur gradually, at intervals of no less than 1 wk. Recommended dose: 5-10 mg/day. Max dose: 15 mg/day. Tourette's disorder 6-18 yr Initial dose 2 mg/day. Gradual dose adjustments up to 5 mg/day at intervals of no less than 2 wk. Recommended dose: 2-20 mg/day. Max dose: 20 mg/day.
Administration
May be taken with or without food.
Special Precautions
Patients w/ dementia-related psychosis. Increased risk of suicidality. Neuroleptic malignant syndrome, tardive dyskinesia, hyperglycemia, DM, orthostatic hypotension, seizures/convulsions, dysphagia. May disrupt body's ability to reduce core body temp. May impair ability to drive or operate machinery. Pregnancy & lactation.
Adverse Reactions
Nausea, vomiting, constipation, headache, dizziness, akathisia, anxiety, insomnia, restlessness, somnolence, extrapyramidal disorder, sedation, tremor, fatigue, blurred vision, salivary hypersecretion.
Drug Interactions
May enhance the effect of certain antihypertensives. Other centrally acting drugs or alcohol, carbamazepine, ketoconazole, quinidine, fluoxetine or paroxetine.
