Indications/Uses
Initial & recurrent mucosal & cutaneous Herpes simplex (HSV-1 & HSV-2) & varicella-zoster (shingles) infections in immunocompromised patients. Herpes simplex encephalitis in patients >6 mth & severe initial clinical episodes of herpes genitalis in non-immunocompromised patients.
Dosage/Direction for Use
Slow IV infusion Mucosal & cutaneous herpes simplex (HSV-1 & HSV-2) infections Immunocompromised patients: Adult 5 mg/kg infused at a constant rate over 1 hr every 8 hr (15 mg/kg/day) for 7 days. Childn <12 yr 250 mg/m2 by infusion at a constant rate over 1 hr every 8 hr for 7 days. Herpes simplex encephalitis 10 mg/kg infused at a constant rate over at least 1 hr every 8 hr for 10 days. Childn 6 mth & 12 yr 500 mg/m2 infused at a constant rate over at least 1 hr every 8 hr for 10 days. Varicella zoster infections Immunocompromised patients 10 mg/kg infused at a constant rate over 1 hr every 8 hr for 7 days. Childn <12 yr 500 mg/m2 at a constant rate over at least 1 hr every 8 hr for 7 days. Obese patients 10 mg/kg. Max: 500 mg/m2 every 8 hr.
Special Precautions
For slow IV infusion only. Adequate hydration. Establish sufficient urine flow w/in first 2 hr following infusion (>500 mL/g). Risk of cerebral edema in patients w/ encephalitis. Encephalothic changes. Pregnancy & lactation.
Adverse Reactions
Fever, pain; elevated liver function tests, nausea; leukopenia, thrombocytopenia; agitation, coma, confusion, convulsions, delirium, hallucinations, obtundation, psychosis; alopecia, erythema multiforme, pruritus, rash, Stevens-Johnson syndrome, toxic epidermal necrolysis, urticaria; elevated BUN, creatinine, renal failure.
