Alfacalcidol
Indications/Uses
Conditions caused by disturbances of Ca metabolism due to impaired 1-α hydroxylation (chronic renal failure).
Dosage/Direction for Use
Initial dose: Adult & childn >20 kg body wt 1 mcg daily. Childn <20 kg body wt 0.05 mcg/kg/day. Elderly 0.5 mcg/day.
Administration
Should be taken with food.
Contraindications
Hypersensitivity. Hypercalcaemia, metastatic calcification.
Special Precautions
Patients treated w/ cardioactive glycosides or digitalis. Phosphate-binding agent may be used in patients w/ renal bone disease. Discontinue if hypercalcemia occurs. Concomitant use w/ vit D. Monitor plasma Ca regularly. Periodically determine serum Ca, phosphorus, Mg, alkaline phosphate, creatinine, BUN & plasma protein w/ long-term therapy. Pregnancy & lactation.
Adverse Reactions
Hypercalcaemia, hyperphosphatemia, various skin reactions.
Drug Interactions
Induction of hepatic detoxification enzymes w/ barbiturates or anticonvulsants. Intestinal absorption may be interfered by concomitant use w/ cholestyramine & laxatives eg, mineral oil. Increased risk of hypercalcaemia w/ thiazide diuretics.
