Amlodipine besylate
Indications/Uses
HTN, myocardial ischemia whether due to fixed obstruction (chronic stable angina), vasospasm/vasoconstriction angina (Prinzmetal's or variant angina) of coronary vasculature. Reduce the risk of coronary revascularization & prevention of hospitalization due to angina w/ documented CAD (limited to patients w/o heart failure or ejection fraction <40%).
Dosage/Direction for Use
Elderly & hepatic impairment HTN 2.5 mg daily. Angina 5 mg. Adult 5-10 mg once daily. Childn 6-17 yr 2.5-5 mg once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity.
Special Precautions
Severe aortic stenosis &/or hypertrophic cardiomyopathy, hepatic impairment. Pregnancy & lactation. Childn <6 yr.
Adverse Reactions
Peripheral edema, flushing, palpitation, headache, dizziness, fatigue, somnolence, rash, pruritus, male sexual dysfunction, nausea, abdominal pain, dyspepsia, gingival hyperplasia, muscle cramps, weakness, dyspnea, pulmonary edema.
Drug Interactions
May increase level/effect of amifostine, antihypertensive, CYP1A2 substrate, hypotensive agent, Mg salt, neuromuscular blocker (nondepolarizing), nitroprusside, phenytoin, rituximab, tacrolimus. Level/effect may be increased by α1-blocker, antifungals (azole derivative, systemic), Ca channel blocker (nondihydropyridine), cyclosporin (systemic), CYP3A4 inhibitor (moderate/strong), dasatinib, diazoxide, fluconazole, macrolides, Mg salt, MAOI, pentoxifylline, phosphodiesterase-5 inhibitor, prostacyclin analogue, PI, quinupristin. May decrease level/effect of clopidogrel, quinidine. Level/effect may be decreased by barbiturate, Ca salt, carbamazepine, CYP3A4 inducer (strong), deferasirox, methylphenidate, nafcillin, rifamycin derivative. Increased level w/ grapefruit juice. Decreased level w/ St. John's wort. Avoid dong quai (has estrogenic activity); ephedra, yohimbine, ginseng may worsen HTN. Increased antihypertensive effect w/ garlic.
