Indications/Uses
Haematological & neurological symptoms resulting from vit B12 deficiencies.
Dosage/Direction for Use
Adult Clinical vit B12 deficiency 1 tab daily. Severe cases Initially 2 tab daily. Severe neurological symptoms Initial parenteral application. Elderly Normal dose. Childn & adolescent Alternative to parenteral therapy of vit B12 deficiency Recommended dose: Up to 1,000 mcg daily.
Administration
Should be taken on an empty stomach: Preferably taken in the morning. Swallow whole w/ fluid.
Contraindications
Hypersensitivity. Patients w/ tobacco amblyopia or retrobulbar neuritis in pernicious anaemia. B12-deficient individuals who are at risk for Leber's optic atrophy.
Special Precautions
Patients w/ folic acid deficiency. Galactose intolerance, lactase deficiency, glucose-galactose malabsorption, fructose intolerance or sucrase-isomaltase deficiency. Check for symptoms, reticulocyte & blood count (including Hb & haematocrit levels) & mean corpuscular vol (MCV) 7 days after initiation of treatment. Monitor for symptoms, blood count & MCV at 4-wkly intervals for 1st 3 mth of treatment, & at 6-mthly/annual intervals if patient compliance is good; more frequent controls if lack of patient compliance is suspected. Hepatic impairment. Not to be used for megaloblastic anaemia of pregnancy.
Adverse Reactions
Uncommonly, severe hypersensitivity eg, hives, skin rash or itching.
Drug Interactions
Impaired absorption by PPIs (eg, omeprazole), histamine H2-antagonists (eg, cimetidine), colchicine, neomycin & amino salicylic acid. Attenuated effect in anaemia w/ chloramphenicol. Increased absorption w/ steroid drugs eg, prednisone in patients w/ pernicious anaemia. Induced functional vit B12 deficiency w/ nitrous oxide. Lowered serum levels w/ metformin.
