Indications and Dosage

Topical/Cutaneous
Nail fungal infections, Skin fungal infections

Adult: As 1% cream, powder, soln or gel: Apply thinly onto affected areas once daily, preferably at night for 2-4 wk. For nail infections, longer treatment may be necessary; 40% urea paste may be applied initially to soften the nails.

Contraindications

Hypersensitivity to bifonazole.

Special Precautions

Patient w/ history of hypersensitivity reactions to other imidazole antifungals (e.g. econazole, miconazole, clotrimazole). Pregnancy and lactation.

Adverse Reactions

Burning sensation, contact dermatitis, erythema, pruritus, rash, urticaria, blister, skin exfoliation, eczema, dry skin, skin irritation or maceration, peripheral oedema at admin site. Very rarely, systemic hypersensitivity reactions.

Patient Counseling Information

Avoid contact w/ eyes, nose, mouth or other mucous membranes.

Drug Interactions

Possible increase in INR when used w/ warfarin.

Action

Description: Bifonazole is an imidazole derivative w/ broad antifungal spectrum. It inhibits ergosterol synthesis resulting in structural and functional impairment of the fungal cytoplasmic membrane. It also exhibits some antibacterial activity.
Pharmacokinetics:
Absorption: Absorbed well into infected skin.
Distribution: Penetrates various skin layers including the stratum corneum and stratum papillare.
Excretion: Via urine (45%) and faeces (40%). Elimination half-life: Biphasic: 8 hr and 50 hr.
