Bisoprolol fumarate
Indications/Uses
HTN, CHF, angina pectoris.
Dosage/Direction for Use
Adult 2.5-5 mg once daily. May be increased to 10 mg up to 20 mg once daily if necessary. Elderly Initially 2.5 mg/day. May be increased to 2.5-5 mg/day. Max: 20 mg/day. 
Administration
May be taken with or without food.
Contraindications
Acute heart failure or during episodes of heart failure decompensation requiring IV inotropic therapy, cardiogenic shock, 2nd or 3rd degree AV block, sick sinus syndrome, SA block, symptomatic bradycardia, symptomatic hypotension, severe bronchial asthma or COPD, severe forms of peripheral arterial occlusive disease or Raynaud's syndrome, untreated pheochromocytoma, metabolic acidosis.
Special Precautions
DM w/ large fluctuations in blood glucose values, severely impaired renal & hepatic function, restrictive cardiomyopathy, congenital heart disease, hemodynamically significant organic valvular disease, MI w/in 3 mth; during long periods of fasting acidosis. Psoriasis or a history thereof. Symptoms of hypoglycemia & thyrotoxicosis may be masked. Pre-op as cardiac output may be impaired under anesth. Rare hereditary problems of galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. Elderly >80 yr.
Adverse Reactions
Chest pain, fatigue, insomnia, hypoesthesia, diarrhea, nausea, vomiting, arthralgia, asthenia, upper resp infection, rhinitis, sinusitis, dyspnea.
Drug Interactions
Acetylcholinesterase inhibitors, anilopiperidine opioids, AV nodal blockers, α-blockers, class 1 antiarrhythmic drugs & SSRIs.
