Indications/Uses
Treatment of infections caused by susceptible organisms of resp tract, ENT, kidneys & urinary tract, skin, soft tissues, bones & joints, gonorrhoea, abdomen. Sepsis, endocarditis, meningitis. Perioperative prophylaxis in patients at increased risk of infection. Prophylaxis in patients w/ reduced resistance.
Dosage/Direction for Use
Adult & childn >12 yr Uncomplicated gonorrhoea 0.5-1 g single dose IM. Uncomplicated/moderate infections 1-2 g IM or IV 8-12 hrly, daily dose 2-6 g. Severe infections 2-3 g IV 6-8 hrly, daily dose 6-12 g. Childn ≥50 kg Adult dose. Infant & childn <50 kg 50-180 mg/kg/day IV or IM 6-8 hrly. Severe infection eg, meningitis Daily dose may be doubled. Premature & neonate 1-4 wk 75-150 mg/kg/day IV 8 hrly, 0-1 wk 50-100 mg/kg/day IV 12 hrly. Perioperative prophylaxis 1-2 g, 30-60 min pre-op. Caesarean 1 g IV at clamping of umbilical cord followed by 1 g IM or IV, 6 & 12 hr after 1st dose.
Contraindications
Hypersensitivity to cefotaxime & other cephalosporins.
Special Precautions
Hypersensitivity to penicillins & other β-lactams. Pseudomembranous colitis. Renal insufficiency, patient necessitating Na restriction. Monitor for event of neutropenia in case of >10 day treatment. Pregnancy & lactation.
Adverse Reactions
Pain at the inj site (for IM) leukopenia, eosinophilia, thrombocytopenia; Jarisch-Herxheimer reaction; convulsions; diarrhea; increase in liver enzymes; rash, pruritus, urticarial; decrease in renal function; fever; inflammatory reactions at the inj site.
Drug Interactions
Excretion delayed by probenecid. Renal function should be closely monitored in patients under concurrent or subsequent medication w/ potentially nephrotoxic drug (eg, aminoglycosides).
