Indications/Uses
Treatment of status epilepticus, general or partial tonic-clonic crisis. Treatment & prevention of convulsions in neurosurgery. Treatment of ventricular tachycardia, atrial paroxysmal tachycardia & arrhythmias induced by digitalis intoxication.
Dosage/Direction for Use
Status epilepticus & tonic-clonic crisis Adult A loading dose of about 18 mg/kg/24 hr IV, followed by maintenance doses of 5-7 mg/kg/day IV every 6-8 hr. Neonate & childn Initially doses of 15-20 mg/kg IV, therapeutic plasma conc are attained (10-20 mcg/mL). Maintenance doses: 5 mg/kg/24 hr. Neurosurgery Adult Initially 15-18 mg/kg/24 hr divided into 3 doses (½ dose initially, ¼ dose after 8 hr & ¼ dose after 16 hr), to continue w/ maintenance dose of 5-7 mg/kg/24 hr divided into 3 doses ie, at 24, 32 & 40 hr & successively. Neonate & childn Initial doses of 15 mg/kg/24 hr, maintenance doses: 5 mg/kg/24 hr. Arrhythmias 100 mg every 5 min until arrhythmia disappears or max of 1,000 mg is reached. Inj rate should not exceed 25-50 mg/min.
Adverse Reactions
CV collapse, CNS depression. When it is administered too fast, hypotension can occur. Atrial & ventricular conduction, depression & ventricular fibrillation. Nystagmus, ataxia, speech difficulties, coordination disorders & mental confusion, dizziness, insomnia, nervousness & headache. GI disturbances, rash, sometimes w/ fever, exfoliative dermatitis, lupus erythematosus & Stevens-Johnson syndrome.
