Dexmedetomidine HCl

Indications/Uses

Sedation of initially intubated & mechanically ventilated patients during treatment in an intensive care setting; non-intubated patients prior to &/or during surgical & other procedures.

Dosage/Direction for Use

IV infusion Individualized dosage. Adult ICU sedation Loading dose: 1 mcg/kg loading infusion over 10 min. Maintenance infusion: 0.2-0.7 mcg/kg/hr. Procedural sedation Loading dose: 1 mcg/kg loading infusion over 10 min. Less invasive procedures (eg, ophth surgery): 0.5 mcg/kg loading infusion over 10 min. Maintenance infusion: 0.6 mcg/kg/hr & titrated to desired clinical effect w/ dose ranging from 0.2-1 mcg/kg/hr. Patients >65 yr Initially 0.5 mcg/kg loading infusion over 10 min. Awake fiberoptic intubation 1 mcg/kg loading infusion over 10 min. Maintenance infusion: 0.7 mcg/kg/hr until the endotracheal tube is secured.

Overdosage

Contraindications

Hypersensitivity. Advanced heart block (Grade 2 or 3) unless paced. Uncontrolled hypotension. Acute cerebrovascular conditions.

Special Precautions

Advanced heart block &/or severe ventricular dysfunction. Hypotension &/or bradycardia in patients w/ hypovolemia, DM or chronic HTN & elderly. Concomitant use w/ vasodilators or negative chronotropic agents. Transient HTN during loading dose. Discontinue if tachycardia &/or HTN occurs & administer supportive therapy. Prolonged exposure (>24 hr). Hepatic impairment. Pregnancy & lactation.

Adverse Reactions

Bradycardia; hypotension, HTN. Hyperglycaemia, hypoglycaemia; agitation; myocardial ischaemia or infarction, tachycardia; resp depression; nausea, vomiting, dry mouth; w/drawal syndrome, hyperthemia.
