Irbesartan
Indications/Uses
Essential HTN. Renal disease in patients w/ HTN & type 2 DM as part of antihypertensive drug regimen.
Dosage/Direction for Use
HTN & HTN w/ type 2 DM 150 mg once daily may be increased to 300 mg once daily. Max: 300 mg once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Pregnancy (2nd & 3rd trimester).
Special Precautions
Correct intravascular vol depletion prior to administration. Patients w/ bilateral renal artery stenosis. Renal & hepatic impairment. Kidney transplant. Monitor serum K in patients at risk of hyperkalemia. Pregnancy.
Adverse Reactions
Hyperkalemia. Orthostatic hypotension, fatigue, dizziness, diarrhea, dyspepsia, upper resp infection, cough.
Drug Interactions
Increased risk of hyperkalemia w/ K salts/supplements, clotrimazole, ACE inhibitors & K-sparing diuretics. Increased effects of amiodarone, bosentan, dapsone, fluoxetine, glimepiride, glipizide, losartan, montelukast, nateglinide, paclitaxel, phenytoin, pioglitazone, repaglinide, rosiglitazone, warfarin, zafirlukast & other CYP2C8 & CYP2C9 substrates, & CYP2D6 (weak) & CYP3A4 (weak). Dong quai. HTN may worsen w/ ephedra, yohimbine, ginseng. Increased antihypertensive effect w/ garlic.
