Dosage/Direction for Use
HTN Adult Initially 50 mg once daily; may be administered once or bid w/ total daily dose range of 25-100 mg. Childn 6-16 yr 0.7 mg/kg once daily; max: 50 mg/day. Patients receiving diuretics or w/ intravascular vol depletion Initially 25 mg. Nephropathy in patients w/ type 2 DM & HTN Adult Initially 50 mg once daily; may be increased to 100 mg once daily based on BP response. Stroke reduction (HTN w/ left ventricular hypertrophy) Adult 50 mg once daily; max: 100 mg/day. May be used in combination w/ thiazide diuretic.
Administration
May be taken with or without food.
Contraindications
Bilateral renal artery stenosis. Concomitant use w/ aliskiren (renin inhibitor) in patients w/ DM. Pregnancy (2nd & 3rd trimesters).
Special Precautions
Vol-depleted patients. Preexisting renal insufficiency or significant aortic/mitral stenosis. Unilateral or bilateral renal artery stenosis, CrCl <30 mL/min. Hepatic dysfunction. Pregnancy.
Adverse Reactions
Chest pain, fatigue, hypoglycemia, diarrhea, UTI, anemia, weakness, back pain, cough.
Drug Interactions
Cimetidine, K salts/supplements, co-trimoxazole (high dose), ACE inhibitors, amiloride, spironolactone, triamterene, lithium, amiodarone, fluoxetine, glimepiride, glipizide, nateglinide, phenytoin, pioglitazone, rosiglitazone, sertraline, warfarin, other CYP2C8/9 substrates, fluconazole, aminoglutethimide, carbamazepine, nafcillin, nevirapine, phenobarb, rifampin, rifapentine, secobarbital, other CYP2C8/9 or 3A4 inducers, NSAIDs.
