Metformin HCl
Indications/Uses
Treatment of type 2 or maturity onset DM not responsive to diet alone or diet plus treatment w/ sulphonylurea.
Dosage/Direction for Use
Metformin 500 Initially 1 tab bid, gradually increased to 1 tab wkly. Metformin 850 1 tab daily, gradually increased to 2 tab every 2 wk. Max: 2.5 g daily in 3 divided doses.
Administration
Should be taken with food.
Contraindications
Impaired hepatic or renal function, CV collapse, CHF, acute MI or conditions leading to hypotension or hypoxaemia. Patients w/ DM complicated by acidosis, infection or gangrene. Surgery. Pregnancy.
Special Precautions
Lactic acidosis. Discontinue 48 hr before elective surgery w/ general anesth or prior to or at the time of administration of iodinated contrast agent. Renal impairment; determine serum creatinine levels before initiating treatment & monitor regularly. Elderly. Childn 10-12 yr. Lactation.
Adverse Reactions
Anorexia, nausea, vomiting, flatulence, dyspepsia & diarrhoea. Metallic taste, wt loss, weakness, lassitude or skin rashes. Lactic acidosis w/ possible abdominal pain, vertigo, palpitation, diminished consciousness, dyspnea.
Drug Interactions
Reduces absorption of vit B12. Increased risk of lactic acidosis w/ alcohol. Glucocorticoids, β2-agonists, diuretics; ACE inhibitors.
