Micardis Telmisartan. Micardis Plus 40/12.5 mg tab Telmisartan 40 mg, hydrochlorothiazide 12.5 mg. Micardis Plus 80/12.5 mg tab Telmisartan 80 mg, hydrochlorothiazide 12.5 mg
Treatment of essential HTN. Prevention of CV morbidity & mortality in patients ≥55 yr at high risk of CV disease. Micardis Plus: Also for patients whose BP is not adequately controlled on Micardis or hydrochlorothiazide alone.
Dosage/Direction for Use
Micardis Essential HTN 40 mg once daily. Max: 80 mg once daily. May be combined w/ thiazide-type diuretics. Prevention of CV morbidity & mortality 80 mg once daily. Micardis Plus Take once daily. Severe HTN Max: 160 mg alone & w/ hydrochlorothiazide 12.5-25 mg/day. Max effect attained 4-8 wk after start of treatment.
May be taken with or without food.
Hypersensitivity. Cholestasis & biliary obstructive disorders, severe hepatic impairment. Fructose intolerance. Pregnancy (2nd & 3rd trimesters) & lactation. Childn <18 yr. Micardis Plus: Severe renal impairment (CrCl <30 mL/min). Refractory hypokalaemia, hypercalcaemia.
Special Precautions
Bilateral renal artery stenosis or stenosis of the artery to a single functioning kidney. Impaired hepatic function. Patients who are vol- &/or Na-depleted by vigorous diuretic therapy, dietary salt restriction, diarrhoea or vomiting. Monitor serum K in patients at risk of hyperkalaemia. Severe CHF; primary aldosteronism; aortic or mitral stenosis, obstructive hypertrophic cardiomyopathy; ischemic cardiopathy or CV disease. Micardis Plus: May impair glucose tolerance. Perform periodic test of serum electrolytes. Increased risk of developing SLE, acute myopia & angle-closure glaucoma.
Adverse Reactions
UTI, upper resp tract infection, sepsis including fatal outcome, anaemia, eosinophilia, thrombocytopenia, anaphylactic reaction, hypersensitivity, hyperkalaemia, hypoglycaemia, insomnia, depression, anxiety, syncope (faint), visual disturbances, vertigo, bradycardia, tachycardia, hypotension, orthostatic hypotension, dyspnoea, GI disorders, abnormal hepatic function/liver disorder, pruritus, hyperhidrosis, rash, angioedema, eczema, erythema, urticaria, drug eruption, toxic skin eruption, back pain, muscle spasm, myalgia, arthralgia, leg pain & tendinitis-like symptoms, renal impairment eg, acute renal failure, chest pain, asthenia, flu-like illness, increased uric acid, blood creatinine, liver enzymes & CPK, decreased Hb. Micardis Plus: Exacerbation of SLE, hypokalaemia, hyponatraemia, hyperuricaemia, dizziness, paresthesia, sleep disturbances, arrhythmias, resp distress, impotence.
Drug Interactions
Monitor digoxin & lithium levels during concomitant use. Reduced effect w/ NSAIDs. Micardis Plus: Other kaliuretic diuretics, laxatives, corticosteroids, ACTH, amphotericin, carbenoxolone, penicillin G Na, salicylic acid & derivatives.
