Indications/Uses
Dosage/Direction for Use
Central diabetes insipidus Tab Adult & childn 0.1-0.2 mg tid. Intranasal rhinyle & nasal spray Adult 10-20 mcg once-bid. Childn 5-10 mcg once-bid. IV inj Adult 1-4 mcg (0.25-1 mL) once-bid. Childn >1 yr 0.4-1 mcg (0.1-0.25 mL) once-bid. Infant <1 yr 0.2-0.4 mcg (0.05-0.1 mL) once-bid. Primary nocturnal enuresis Tab 0.2-0.4 mg nocte. Nocturia Tab Initially, 0.1 mg at bedtime. If not sufficiently effective after 1 wk, increase to 0.2 mg then 0.4 mg wkly. Renal conc capacity test Intranasal rhinyle & nasal spray Adult 40 mcg. Childn >1 yr 20 mcg. Infant <1 yr 10 mcg. IM/SC inj Adult 4 mcg. Childn >1 yr 1-2 mcg. Infant <1 yr 0.4 mcg. Therapeutic control of bleeding & bleeding prophylaxis in Haemophilia A & von Willebrand's disease Inj 0.3 mcg/kg by IV infusion over 15-30 min. Adult & childn ≥10 kg Use 50 mL of diluent. Childn <10 kg Use 10 mL of diluent. If a positive effect is obtained, the initial dose may be repeated 1-2 times w/ interval of 6-12 hr.
Administration
May be taken with or without food.
Contraindications
Habitual & psychogenic polydipsia, syndrome of inappropriate antidiuretic hormone secretion (SIADH), hyponatraemia, moderate to severe renal insufficiency. For renal conc capacity test: Cardiac insufficiency & other conditions requiring treatment w/ diuretics. For hemostatic use: Unstable angina pectoris, decompensated cardiac insufficiency, von Willebrand's disease type II B.
Special Precautions
Avoid fluid overload in the very young & elderly, conditions characterized by fluid &/or electrolyte imbalance & those at risk for increased intracranial pressure. Caution in renal conc capacity testing for childn <1 yr. In hemostatic use, avoid fluid overload in patient taking diuretics. When used for diagnostic purposes, limit fluid intake to max 0.5 L 1 hr before until 8 hr after administration. Monitor fluid retention & Na plasma levels if used w/ drugs known to induce SIADH & NSAIDs. Pregnancy. Nasal spray Severe hyponatraemia when used in the treatment of cranial diabetes insipidus. Contains benzalkonium Cl, may cause bronchospasm.
Adverse Reactions
Transient headache, nausea, stomach pain, epistaxis & fatigue, transient fall in BP w/ reflex tachycardia & facial flushing, nasal congestion/rhinitis, epistaxis. Rarely, dizziness.
Drug Interactions
Magnitude but not the duration of response may be augmented by indomethacin. TCA, SSRIs, chlorpromazine & carbamazepine may increase antidiuretic hormone secretion. NSAIDs.

H01BA02 - desmopressin ; Belongs to the class of vasopressin and analogues. Used in posterior pituitary lobe hormone preparations.
