Metronidazole
Indications/Uses
Serious infections eg, intra-abdominal, gynecologic, skin & skin structure, bone & joints, lower resp, CNS infections; septicemia & endocarditis caused by susceptible anaerobic bacteria. Perioperative prophylaxis in patients undergoing contaminated or potentially contaminated elective colorectal surgery.
Dosage/Direction for Use
IV infusion Loading dose: 15 mg/kg infused over 1 hr. Maintenance dose: 7.5 mg/kg infused over 1 hr 6 hrly. Max: 4 g during 24-hr period. Perioperative prophylaxis 15 mg/kg infused over 30-60 min, followed by 7.5 mg/kg infused over 30-60 min at 6 & 12 hr after initial dose.
Contraindications
Hypersensitivity to metronidazole or other nitroimidazole derivatives.
Special Precautions
Discontinue treatment when peripheral neuropathy occurs in prolonged therapy. Patients receiving corticosteriods; w/ renal & CV disease, predisposed to edema. Superinfection or suprainfection. Blood dyscrasia. May impair ability to drive or operate machinery. Severe hepatic disease. Pregnancy & lactation. May interfere w/ certain types of determinations of serum chemistry values eg, AST, ALT, lactate dehydrogenase (LDH), triglycerides & hexokinase glucose.
Adverse Reactions
Convulsive seizures, peripheral neuropathy. Nausea, vomiting, abdominal discomfort, diarrhea, unpleasant metallic taste; reversible leukopenia; erythematous rash, pruritus; headache, dizziness, syncope, ataxia, confusion; thrombophlebitis; fever, darkened urine, furred tongue, glossitis & stomatitis.
Drug Interactions
Potentiates anticoagulant effects of warfarin & other oral coumarin anticoagulants. Reduced plasma levels & efficacy w/ phenobarb. Increased plasma t½ & plasma level w/ cimetidine. Disulfiram-like reactions (eg, symptoms of flushing, palpitation, tachycardia, nausea, vomiting) may occur w/ alcoholic beverages. Acute psychosis or confusion may occur w/ disulfiram. May decrease elimination of anticonvulsants, hydantoin & lithium. May increase hydantoin & lithium conc.
