Indications/Uses
Duodenal & gastric ulcer, moderate & severe reflux esophagitis, Zollinger-Ellison syndrome & other pathological hypersecretory conditions.
Dosage/Direction for Use
IV Duodenal & gastric ulcer, moderate & severe reflux esophagitis Recommended dose: 40 mg/day. Long-term management of Zollinger-Ellison syndrome & other hypersecretory conditions Initially 80 mg daily, thereafter, dosage can be titrated up or down as needed using measurements of gastric acid secretion to guide. Doses >80 mg daily, should be given in 2 divided doses daily. Rapid acid control Initially 2 x 80 mg to decrease acid output into the target range (<10 mEq/hr) w/in 1 hr.
Contraindications
Special Precautions
Moderate to severe hepatic function & impaired renal function. Risk of bone fracture & C. difficile infection. Hypomagnesemia. Monitor regularly liver enzymes during therapy & prolonged use. Concomitant use w/ HIV PIs & methotrexate. Gastric malignancy. Patients w/ reduced body stores or risk factors for reduced vit B12 absorption on prolonged therapy. May impair ability to drive or operate machinery. Pregnancy & lactation. Childn <18 yr.
Adverse Reactions
Drug Interactions
Reduce bioavailability of HIV PIs eg, atazanavir, nelfinavir. May interfere w/ absorption of other drugs w/ pH-dependent absorption pharmacokinetics. May elevate & prolong serum levels of methotrexate. May increase whole blood level of tacrolimus. Increased systemic exposure w/ fluvoxamine. Increased INR & prothrombin time w/ warfarin or phenprocoumon.
