Pentamidine isethionate
Indications/Uses
Pneumocystis carinii (Pneumocystis jiroveci) pneumonia.
Dosage/Direction for Use
Oral inhalation Pneumocystis carinii pneumonia prophylaxis Adult & childn ≥5 yr 300 mg as a single inhalation every 4 wk. Infusion Adult, childn & infant ≥4 mth 4 mg/kg body wt/day for at least 14 days. Max: 21 days.
Contraindications
Hypersensitivity.
Special Precautions
HTN, hypotension, hyperglycaemia, hypoglycaemia, hypocalcaemia, leucopenia, thrombocytopenia, impaired hepatic or renal function, CHD, history of ventricular arrhythmias, untreated hypokalemia &/or hypomagnesemia, bradycardia (<50 bpm). Asthmatics. Monitor BP (if given IV), BUN; serum creatinine, serum Ca; CBC, liver function, blood sugar, urinalysis, ECG. Pregnancy, lactation.
Adverse Reactions
Renal undesirable effects, local reactions. Less severe reactions eg azotemia, abnormal liver function tests, anemia, macroscopic hematuria, anorexia, diarrhea, nausea, vomiting, dizziness, collapse, flush, rash, taste disturbances. Severe reactions eg, anaphylactic shock, HTN, hypotension, hypoglycemia, hyperglycemia, DM, pancreatitis, cardiac arrhythmias, QT interval prolongation, leucopenia, neutropenia, thrombocytopenia, acute renal failure, hypomagnesemia, hyperkalemia, hypocalcemia. Azotemia, abnormal liver function test, leucopenia, anemia, thrombocytopenia, macroscopic hematuria, anorexia, diarrhea, nausea, vomiting, dizziness, collapse, flush, rash & taste disturbances.
Drug Interactions
Increased risk of pancreatitis w/ concomitant didanosine or zalcitabine. Foscarnet, amphotericin B, drugs that prolong QT interval.
