Indications/Uses
Central & peripheral neuropathic pain in adults which includes diabetic peripheral neuropathy & post herpetic neuralgia. Adjunctive therapy in adults w/ partial seizures w/ or w/o secondary generalization. Management of fibromyalgia.
Dosage/Direction for Use
150-600 mg daily in 2 or 3 divided doses. Adult Neuropathic pain Initially 150 mg once daily. May be increased to 300 mg once daily after 3-7 days interval. Max: 600 mg once daily after an additional 7-day interval. Epilepsy Initially 150 mg once daily. May be increased to 300 mg once daily after 1 wk. Max: 600 mg once daily after an additional wk interval. Fibromyalgia Usual dose: 300-450 mg once daily in 2-3 divided doses. Initially 75 mg bid, & may be increased to 150 mg bid w/in 1 wk based on efficacy & tolerability. May be further increased to 225 mg bid if no sufficient benefit is experienced w/ 300 mg/day. Max: 600 mg once daily after an additional wk. Renal impairment CrCl ≥60 mL/min 150 mg/day bid or tid. Max: 600 mg/day bid or tid; ≥30 to <60 mL/min 75 mg/day bid or tid. Max: 300 mg/day bid or tid; ≥15 to <30 mL/min 25-50 mg/day once daily or bid. Max: 150 mg/day once daily or bid; <15 mL/min 25 mg/day once daily. Max: 75 mg/day once daily.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Hypersensitivity reactions. History of angioedema episodes. Concomitant use w/ drugs known to cause angioedema eg, ACE inhibitors. Abrupt discontinuation may cause anxiety, diarrhea, headache, hyperhidrosis, insomnia & nausea. May cause hematologic disorder. Do not drive or operate machinery, or ingest alcoholic beverages during therapy. Liver & kidney disease. Pregnancy & lactation.
Adverse Reactions
Dizziness, somnolence, ataxia, tremor, amnesia, speech disturbances, myoclonus, neuropathy, confusion euphoria, incoordination, abnormal gait, nervousness, twitching, headache.
Drug Interactions
Increase levels/effects of alcohol, analgesics (eg, opioid); antidiabetics (eg, thiazolidinedione); azelastine (nasal), blonanserin, buprenorphine, CNS depressants, flunitrazepam, hydrocodone, methotrimeprazine, metyrosine, mirtazapine, orphenadrine, oxycodone, paraldehyde, piribedil, pramipexole, ropinirole, rotigotine, SSRI, suvorexant, thalidomide, zolpidem. Levels/effects may be increased by ACE inhibitors, brimonidine (topical), cannabis, chlormethiazole, chlorophenesin, carbamate, dimethindene (topical), doxylamine, dronabinol, droperidol, hydroxyzine, kava kava, lofexidine, Mg sulfate, methotrimeprazine, minocycline, nabilone, oxomemazine, perampanel, rufinamide, Na oxybate, tapentadol, tetrahydrocannabinol, trimeprazine. Decreased levels/effects by mefloquine, mianserin, orlistat.
