Indications/Uses
Depression in patients w/ or w/o history of mania; obsessive-compulsive disorder (OCD), panic disorder w/ or w/o agoraphobia, post-traumatic stress disorder (PTSD), social phobia, premenstrual dysphoric disorder (PMDD).
Dosage/Direction for Use
Depression & OCD 50 mg/day. Panic disorder, PTSD & social phobia Initially 25 mg/day; increase to 50 mg once daily after 1 wk. PMDD Initially 50 mg/day either daily throughout the menstrual cycle or limited to the luteal phase of menstrual cycle. OCD Childn 13-17 yr Initially 50 mg/day, 6-12 yr Initially 25 mg/day, increased to 50 mg/day after 1 wk. Max: 200 mg/day. Elderly Same as childn dose. All doses to be given once daily either in the morning or evening.
Administration
May be taken with or without food.
Contraindications
Concomitant MAOIs & pimozide.
Special Precautions
Co-administration w/ serotonergics. Seizures, anti-obsessional, hepatic disease. Avoid alcohol. May impair ability to drive or operate machinery. Pregnancy, lactation.
Adverse Reactions
GI, nervous system, psychiatric, hepatobiliary, skin & subcutaneous tissue disorders. Anorexia, increased appetite, hyponatremia, sexual dysfunction, galactorrhea, gynecomastia, menstrual irregularities, priapism, leucopenia, thrombocytopenia, palpitations, tachycardia, tinnitus, hyperprolactinemia, hypothyroidism, syndrome of inappropriate ADH secretion, mydriasis, abnormal vision, asthenia, chest pain, peripheral edema, fatigue, fever, malaise, allergic/anaphylactoid reaction, arthralgia, muscle cramps, urinary incontinence/retention, bronchospasm, yawning, abnormal bleeding (eg, epistaxis, GI bleeding or hematoma), hot flushes, HTN, abnormal lab tests.
Drug Interactions
Lithium, phenytoin, sumatriptan, protein-bound drugs, warfarin, diazepam, tolbutamide, cimetidine, TCAs, propafenone, flecainide, tryptophan, fenfluramine or 5-HT agonists.
