Indications/Uses
Anti-inflammatory or immunosuppressant agent in the treatment of hematologic, allergic, inflammatory, neoplastic & autoimmune origin diseases.
Dosage/Direction for Use
Anti-inflammatory or immunosuppressive Adult IM 10-80 mg/day once daily. IV 10-40 mg over a period of several min & repeated IV or IM at intervals depending on clinical response. Childn IM/IV 0.5-1.7 mg/kg/day or 5-25 mg/m2/day in divided doses every 6-12 hr. Status asthmaticus Adult & childn IV Loading dose: 2 mg/kg/dose then, 0.5-1 mg/kg/dose 6 hrly up to 5 days. Acute spinal cord injury Adult & childn IV 30 mg/kg over 15 min, followed in 45 min by a continuous infusion of 5.4 mg/kg/hr for 23 hr. Lupus nephritis Adult High dose: IV 1 g/day for 3 days. Childn IV 30 mg/kg over ≥30 min every other day for 6 doses.
Contraindications
Hypersensitivity. Viral, fungal or tubercular skin lesions. Administration of live virus vaccines. Serious infections except septic shock or tuberculous meningitis.
Special Precautions
Hyperthyroidism, cirrhosis, nonspecific ulcerative colitis, HTN, osteoporosis, CHF, convulsive disorders, myasthenia gravis, thrombophlebitis, peptic ulcer, diabetes, glaucoma, cataracts or TB. Hepatic impairment. Abrupt w/drawal after prolonged use or stress may cause acute adrenal insufficiency. Childn susceptible to adrenal axis suppression. Elderly. Pregnancy & lactation.
Adverse Reactions
Edema, HTN, arrythmias; insomnia, vertigo, seizures, headache, mood swings, delirium, hallucinations; hirsutism, acne, skin atrophy, hyperpigmentation; DM, adrenal suppression, hyperlipidemia, Cushing's syndrome, pituitary adrenal axis suppression, growth suppression, glucose intolerance, hypokalemia, alkalosis, amenorrhea, Na & water retention, hyperglycemia; increased appetite, indigestion, peptic ulcer, nausea, vomiting, abdominal distention, ulcerative esophagitis; transient leukocytosis; muscle weakness, osteoporosis; cataracts, glaucoma; infections, hypersensitivity reactions, secondary malignancy, intractable hiccups.
Drug Interactions
May increase circulating glucose levels, cyclosporine & tacrolimus blood levels. Itraconazole increases corticosteroid levels. Increased clearance w/ phenytoin, phenobarb & rifampin. Decreased antibody response w/ skin test antigens & immunizations.
