Theophylline (anhydrous)
Indications/Uses
Symptomatic relief &/or prevention of bronchospasm associated w/ asthma, chronic bronchitis & emphysema.
Dosage/Direction for Use
Adult Initially 16 mg/kg or 400 mg in 2 divided doses then gradually increase up to 800 mg/day. Childn ≥16 yr 13 mg/kg or 900 mg daily, 12-16 yr 18 mg/kg daily, 9-12 yr 20 mg/kg daily, up to 9 yr 24 mg/kg daily.
Administration
May be taken with or without food: May be taken w/ meals to reduce GI discomfort.
Contraindications
Hypersensitivity to xanthine derivatives. Active PUD, seizure disorders.
Special Precautions
Pregnancy & lactation. Childn.
Adverse Reactions
Nausea, vomiting, headache & insomnia. Persistent vomiting, cardiac arrhythmias & intractable seizures which can be lethal. Diarrhoea, irritability, restlessness, fine skeletal muscle tremors & transient diuresis.
Drug Interactions
Increased excretion & decreased therapeutic effectiveness of lithium. May enhance sensitivity & toxic potential of cardiac glycosides. May exhibit synergistic toxicity w/ ephedrine & other sympathomimetics. Enhanced effects of oral anticoagulants. Increased serum conc w/ cimetidine, high-dose allopurinol, OCs, propranolol, ciprofloxacin, erythromycin & troleandomycin. Decreased serum conc w/ rifampin & phenytoin. Decreased clearance w/ methotrexate. Increase cardiotoxic effects w/ α-adrenergic antagonist eg, isoproterenol.
