Donepezil HCl
Indications/Uses
Symptomatic treatment of mild to severe Alzheimer's disease & vascular dementia (dementia-associated w/ cerebrovascular disease); dementia w/ Lewy bodies (DLB).
Dosage/Direction for Use
Adult & elderly Initially 5 mg/day in the evening. May be increased to 10 mg/day after at least 4-6 wk of clinical assessment in patients who tolerated treatment at 5 mg/day. Max: 10 mg/day.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to donepezil HCl or piperidine. Pregnancy.
Special Precautions
May exaggerate succinylcholine-type muscle relaxation during anaesth. Sick-sinus syndrome or supraventricular cardiac conduction conditions (eg, SA or AV block). Consider possibility of heart block or long sinusal pauses. May cause bladder outflow obstruction; exacerbate or induce extrapyramidal symptoms. History of asthma or obstructive pulmonary disease, & ulcer disease or those receiving concurrent NSAIDs. Concomitant use w/ acetylcholinesterase, agonists or antagonists of the cholinergic system. Severe hepatic impairment. May impair ability to drive or operate machinery. Lactation. Childn.
Adverse Reactions
Diarrhea, muscle cramps, fatigue, nausea, vomiting, insomnia, headache. Common cold; anorexia; hallucinations, agitation, aggressive behavior; syncope, dizziness; abdominal disturbance; rash, pruritis; urinary incontinence; pain; accident.
Drug Interactions
Metabolism may be inhibited by ketoconazole, guanidine, itraconazole, erythromycin, fluoxetine. Levels may be reduced by rifampicin, phenytoin, carbamazepine, alcohol. May interfere w/ drugs having anticholinergic activity (eg, atropine). Synergistic activity may occur w/ succinylcholine, other neuromuscular blockers or cholinergic agonists (eg, bethanechol) or β-blockers which have effects on cardiac conduction or aspirin/NSAIDs (eg, ibuprofen), or naproxen may increase risk of developing stomach ulcers.
