Indications/Uses
Central & peripheral neuropathic pain in adults which includes diabetic peripheral neuropathy & post herpetic neuralgia. Adjunctive therapy in adults w/ partial seizures w/ or w/o secondary generalization. Generalized anxiety disorder (GAD) in adults. Fibromyalgia.
Dosage/Direction for Use
150-600 mg daily in 2 or 3 divided doses. Adult Neuropathic pain Initially 150 mg/day, may be increased to 300 mg/day after an interval of 3-7 days. Max: 600 mg/day after an additional 7-day interval. Epilepsy Initially 150 mg/day, may be increased to 300 mg/day after 1 wk. Max: 600 mg/day after an additional wk. Generalized anxiety disorder 150-600 mg daily in 2 or 3 divided doses, then reassessed regularly. Initially 150 mg daily, may be increased to 300 mg daily w/in 1 wk. If insufficient, increase to 450 mg daily after 1 wk. Max: 600 mg daily after an additional wk. Fibromyalgia 300-450 mg daily. Initially 150 mg daily (75 mg bid), may be increased to 300 mg daily (150 mg bid) w/in 1 wk. If insufficient, increase 300 mg daily to max dose of 450 mg daily (225 mg bid). Max: 600 mg/day after an additional wk. Patients w/ renal impairment CrCl ≥60 mL/min Initially 150 mg/day to a max of 600 mg/day bid or tid, ≥30 to <60 mL/min Initially 75 mg/day to a max of 300 mg/day bid or tid, ≥15 to <30 mL/min Initially 25-50 mg/day to a max dose of 150 mg/day once daily or bid, <15 mL/min Initially 25 mg/day to a max of 75 mg/day once daily. Supplementary dosage following hemodialysis 25 mg/day as single dose. Max: 100 mg/day as single dose.
Administration
May be taken with or without food.
Contraindications
Special Precautions
May cause hematologic abnormalities. Renal or hepatic impairment. Galactose intolerance, the Lapp lactose deficiency, or galactose malabsorption. Patients w/ diabetes may gain wt during treatment. Discontinue if hypersensitivity or angioedema develops; if myopathy is diagnosed or suspected, or markedly elevated CK (CPK) conc occur. Consider further assessment if visual disturbance persists. Avoid abrupt w/drawal of treatment. NYHA class III or IV CHF. Concomitant use w/ thiazolidinedione antidiabetic agents. Increased risk of suicidal thoughts or behavior. May decrease platelet count & cause PR interval prolongation. Avoid driving or operating machinery during treatment. Pregnancy & lactation.
Adverse Reactions
Dizziness, somnolence. Increased appetite; confusion, disorientation, irritability, euphoric mood, decreased libido, insomnia; ataxia, abnormal coordination, balance disorder, amnesia, disturbance in attention, memory impairment, tremor, dysarthria, paraesthesia, sedation, lethargy, blurred vision, diplopia, vertigo, vomiting, abdominal distention, constipation, dry mouth, flatulence; erectile dysfunction; peripheral oedema, oedema, abnormal gait, feeling drunk, fatigue; increased wt.
Drug Interactions
Increased risk of swelling & hives w/ ACE inhibitors. Additive effect on edema & wt gain w/ thiazolidinediones.
