Indications/Uses
Symptomatic treatment of acute & chronic OA & RA. Ankylosing spondylitis (AS), acute gouty arthritis. Relief of chronic musculo-skeletal pain, including chronic low back pain, acute pain associated w/ dental surgery. Primary dysmenorrhea. Moderate to severe acute post-op pain associated w/ abdominal gynecological surgery.
Dosage/Direction for Use
RA, AS 90 mg once daily. Acute pain 90 mg during acute symptomatic period. Post-op dental pain 90 mg once daily for up to 3 days. Post-op gynecological pain 90 mg once daily for up to 5 days. Initial dose should be administered shortly before surgery. Max: 120 mg once daily.
Administration
May be taken with or without food.
Contraindications
History of hypersensitivity; allergic-type reaction (eg, bronchospasm, acute rhinitis, nasal polyps, angioedema, urticaria) to aspirin or other NSAIDS. CHF (NYHA class II-IV); inflammatory bowel disease; established ischemic heart disease, peripheral artery disease &/or cerebrovascular disease (including those who have recently undergone CABG surgery or angioplasty); persistent uncontrolled HTN. Active peptic ulceration or GI bleeding. Severe hepatic dysfunction (Child-Pugh score >9) & severe renal impairment (CrCl <30 mL/min). Pregnancy & lactation. Childn <16 yr.
Special Precautions
Patients w/ significant risk factors for CV disease eg, HTN, hyperlipidaemia & DM. Avoid in patients w/ history of mild allergic phenomena related to ingestion of other NSAIDs eg, rash; w/ conditions predisposing to GI events (eg, history of peptic ulcer, upper GI disease, ulcerative colitis; smoking; advancing age; concurrent use of aspirin or corticosteroids; alcohol abuse; stress); w/ bleeding disorders (potential exacerbation).
Adverse Reactions
Edema, HTN, palpitations; dizziness, fatigue, headache; bruising; abdominal pain, diarrhea, dyspepsia, flatulence, heartburn, nausea; increased ALT & AST; weakness; alveolar osteitis, flu-like syndrome. SJS, TEN.
Drug Interactions
Increase serum conc of warfarin. Decreased plasma conc w/ rifampicin. May decrease antihypertensive effect of diuretics, ACE inhibitors & AIIAs.  Increase plasma conc of ethinylestradiol.
