Rivastigmine
Indications/Uses
Mild to moderate dementia of the Alzheimer type or those associated w/ Parkinson disease.
Dosage/Direction for Use
Adult Initially 4.6 mg/24 hr. Max dose: 9.5 mg/24 hr. Dose should be increased to 9.5 mg/24 hr after a min of 4 wk treatment. Maintenance dose: 9.5 mg/24 hr.
Contraindications
Hypersensitivity to rivastigmine or other carbamate derivatives (eg, neostigmine, pyridostigmine, physostigmine).
Special Precautions
GI effects. Closely monitor for manifestations of active or occult GI bleeding especially in patients at increased risk (eg, history of ulcer disease, concomitant NSAIDs therapy); loss of appetite or wt loss. Patients w/ sick sinus syndrome or other supraventricular cardiac conduction abnormalities; history of asthma or obstructive pulmonary disease (low wt <50 kg). May cause urinary obstruction. Potential for increased risk of secondary seizures. May exacerbate or induce extrapyramidal symptoms. May cause gradual impairment of ability to drive or operate heavy machinery. Pregnancy & lactation. Childn.
Adverse Reactions
Dizziness, headache; nausea, vomiting, diarrhea, anorexia, abdominal pain. Syncope, HTN; fatigue, insomnia, confusion, depression, anxiety, malaise, somnolence, hallucination, aggressiveness, parkinsonism symptoms worsening, vertigo; dyspepsia, constipation, flatulence, wt loss, eructation, dehydration; UTI; weakness, tremor; rhinitis; diaphoresis, flu-like syndrome.
Drug Interactions
May exaggerate succinylcholine-type muscle relaxation w/ anesth. May interfere activity of anticholinergics. May act synergistically w/ succinylcholine, similar neuromuscular blocking agents or cholinergic agonists eg, bethanechol. Active or occult GI bleeding w/ NSAIDs.
