Vancomycin
Indications/Uses
Severe staphylococcal (including MRSA) infections eg, meningitis, bone infection, septicemia, lower resp tract & skin infections. Severe staphylococcal infection in penicillin-allergic patient.
Dosage/Direction for Use
Adult 500 mg 6 hrly or 1 g 12 hrly IV over 60 min. Childn 10 mg/kg 6 hrly over 60 min. Infant & neonate Initially 15 mg/kg then 10 mg/kg 12 hrly for neonates in 1st wk & 8 hrly for infant 1 wk to 1 mth.
Contraindications
Hypersensitivity.
Special Precautions
Necrosis or severe pain in IM. Renal & hearing impairment. Pregnancy & lactation.
Adverse Reactions
Nephrotoxicity, ototoxicity. Redneck or red man syndrome, inflammation at inj site following rapid administration.
Drug Interactions
Increased risk of ototoxicity w/ loop diuretics, aminoglycoside. Increased risk of nephrotoxic w/ amphotericin B, aminoglycosides, bacitracin, polymyxin B, colistin.
