Tympanocentesis may be done to collect middle ear fluid, followed by culture to identify the pathogen. This is reserved for severe or refractory cases. Tympanometry and acoustic reflectometry may also be used to evaluate middle ear effusion.
Imaging
Imaging is not routinely indicated unless intratemporal or intracranial complications are suspected. A computed tomography (CT) scan of the temporal bones may identify mastoiditis, epidural abscess, sigmoid sinus thrombophlebitis, meningitis, brain abscess, subdural abscess, ossicular disease, and cholesteatoma. Magnetic resonance imaging (MRI) may identify fluid collections, especially in the middle ear.
