If the patient presents with clinical features of hyperandrogenism with chronic anovulation, then other identifiable causes need to be excluded. The causes of amenorrhea that need to be considered and excluded include the following:

  • Primary Amenorrhea
    • Androgen-secreting tumors of the ovary (rare pre-menarche)
    • Congenital adrenal hyperplasia
    • Imperforate hymen
    • Primary ovarian insufficiency (eg Turner syndrome)
  • Secondary Amenorrhea
    • Adrenal dysfunction (eg androgen-secreting tumors of the adrenal gland, Cushing syndrome)
    • Anorexia nervosa
    • Drugs (eg oral contraceptives, estrogens or dopamine agonists)
    • Genital tract abnormalities (eg endometrial scarring, uterine or ovarian tumor)
    • Hyperthyroidism, hypothyroidism
    • Pituitary dysfunction (eg acromegaly, functional hypothalamic amenorrhea, hyperprolactinemia)
    • Pregnancy
    • Severe systemic illness
    • Stress or excessive exercise
