Differential Diagnosis

Secondary pupillary block

  • Uveitis causing posterior synechiae and secluded pupil
  • Lens-related disorders (eg ectopia lentis, malpositioned intraocular lens)
  • Pupillary block due to vitreous prolapse

Anterior pulling mechanism

  • Neovascularization of the iris
  • Epithelial or fibrous ingrowth
  • Iridocorneal endothelial syndrome
  • Contracture of the inflammatory precipitates in uveitis
  • PAS following anterior segment surgery

Posterior pushing mechanism

  • Conditions affecting the posterior segment leading to the forward shift of the lens-iris diaphragm
  • Aqueous misdirection (malignant glaucoma)
