Donepezil HCl
Indications/Uses
Symptomatic treatment of mild to moderately severe Alzheimer's disease.
Dosage/Direction for Use
Adult & elderly Initially 5 mg once daily for at least 1 mth; can be increased to 10 mg once daily after 1 mth of clinical assessment w/ 5 mg/day treatment. Max: 10 mg once daily.
Administration
May be taken with or without food: Take at bedtime.
Contraindications
Hypersensitivity to donepezil, piperidine derivatives, soya & peanut or to any of the excipients.
Special Precautions
May exaggerate succinycholine-type muscle relaxation during anaesth. Patient w/ sick sinus syndrome or other supraventricular cardiac conduction conditions eg, SA or AV block; at increased risk for developing ulcers; w/ history of asthma or obstructive pulmonary disease; severe hepatic impairment. May cause bladder outflow obstruction. Has some potential to cause generalized convulsion. Has potential to exacerbate or induce extrapyramidal symptoms. Discontinue if patient develops symptoms indicative of neuroleptic malignant syndrome. Monitor for muscle pain, tenderness or weakness & darkened urine, particularly if accompanied by malaise or fever. Avoid concomitant use w/ acetylcholinesterase inhibitors, cholinergic system agonists or anatagonists. Rare hereditary problems of galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption. Minor or moderate influence on the ability to drive & use machines. Pregnancy & lactation. Childn & adolescents <18 yr.
Adverse Reactions
Diarrhoea, nausea; headache. Common cold; anorexia; hallucinations, agitation, aggression, nightmares; syncope, dizziness, insomnia; vomiting, abdominal disturbance; rash, pruritis; muscle cramps; urinary incontinence; fatigue, pain.
Drug Interactions
Inhibited metabolism w/ CYP3A4 inhibitors (eg, ketoconazole, itraconazole & erythromycin) & CYP2D6 inhibitors (eg, quinidine & fluoxetine). Reduced levels w/ enzyme inducers eg, rifampicin, phenytoin, carbamazepine & alcohol. May interfere w/ anticholinergic drugs. Potential synergistic activity w/ succinylcholine, neuro-muscular blocking agents or cholinergic agonists or β-blockers.
