Metformin HCl
Indications/Uses
Type 2 DM (NIDDM) in overwt patients when diet & exercise alone have not been enough to control blood glucose levels.
Dosage/Direction for Use
Adult Initially 500 or 850 mg bd-tds. Max dose: 3,000 mg/day taken as 3 divided doses. Childn & adolescents >10 yr Initially 500 or 850 mg once daily. Max dose: 2,000 mg/day taken as 2 or 3 divided doses. Renal impairment w/ GFR 60-89 mL/min Total max dose: 3,000 mg/day in 2-3 divided doses, GFR 45-59 mL/min Total max dose: 2,000 mg/day in 2-3 divided doses, GFR 30-44 mL/min Total max dose: 1,000 mg/day in 2-3 divided doses.
Administration
Should be taken with food.
Contraindications
Hypersensitivity. Uncontrolled diabetes w/ severe hyperglycaemia, nausea, vomiting, diarrhoea, rapid wt loss, lactic acidosis or ketoacidosis. Dehydration. Excessive alcohol intake. Acute heart failure or recent heart attack, severe circulatory problems (eg, shock) or breathing difficulties. Severe infections. Liver problems. Severely reduced kidney function.
Special Precautions
Risk of lactic acidosis. Discontinue use in case of conditions that may be associated w/ dehydration; symptoms of lactic acidosis; major surgery. Intravascular administration of iodinated contrast agents. Risk of hypoglycaemia w/ other antidiabetics (eg, sulphonylureas, insulin, meglitinides). Renal impairment. Pregnancy & lactation.
Adverse Reactions
Digestive problems (eg, nausea, vomiting, diarrhoea, abdominal pain & loss of appetite). Changes in taste.
Drug Interactions
Diuretics; NSAIDs & COX-2 inhibitors (eg, ibuprofen & celecoxib); ACE inhibitors & AIIA; β2 agonists (eg, salbutamol or terbutaline); corticosteroids; medicines that may change the amount of Brot in the blood, especially if w/ reduced kidney function (eg, verapamil, rifampicin, cimetidine, dolutegravir, ranolazine, trimethoprime, vandetanib, isavuconazole, crizotinib, olaparib); other antidiabetics. Increased risk of lactic acidosis w/ excessive alcohol intake.
