Azilsartan medoxomil 40 mg, chlorthalidone 12.5 mg
Dosage/Direction for Use
Initially, one 40/12.5 mg tab once daily. Dose may be increased to 40/25 mg after 2-4 wk.
Administration
May be taken with or without food.
Contraindications
Patient w/ anuria. Do not co-administer w/ aliskiren-containing products in patients w/ DM or renal impairment (GFR <60 mL/min/1.73 m2).
Special Precautions
Hypotension in vol- or salt-depleted patients. Monitor for worsening renal function in patients w/ renal impairment. Monitor serum electrolytes periodically. Dual blockage of the renin-angiotensin-aldosterone system (RAAS) through combined use of ACE inhibitors, angiotensin II receptor blockers or aliskiren. Concomitant use w/ ACE inhibitors or angiotensin II receptor blockers in patients w/ diabetic nephropathy. Pregnancy & lactation. Childn <18 yr. Chlorthalidone: Hyperuricemia. May precipitate azotemia. Minor alterations of fluid & electrolyte balance may precipitate hepatic coma in patients w/ impaired hepatic function or progressive liver disease.
Adverse Reactions
Dizziness, fatigue. Azilsartan: Diarrhea, nausea, asthenia, muscle spasm, dizziness postural, cough. Chlorthalidone: Rash, headache, GI upset & elevations of uric acid & cholesterol.
Drug Interactions
In elderly, vol-depleted patients or w/ compromised renal function, co-administration of NSAIDs, including selective COX-2 inhibitors, w/ angiotensin II receptor antagonists (including azilsartan) may result in deterioration of renal function, including possible acute renal failure. Attenuated antihypertensive effect w/ NSAIDs. Dual blockade of the renin angiotensin system w/ angiotensin receptor blockers, ACE inhibitors or aliskiren is associated w/ increased risks of hypotension, hyperkalemia & changes in renal function (including acute renal failure). Increased risk of lithium toxicity.
