Ispaghula husk, ground (Plantago ovata seed shells)
Indications/Uses
Persistent (chronic) constipation. Conditions in which easier bowel movements w/ soft stools are desirable eg, anal fissures, haemorrhoids, painful bowel movements following rectal or anal surgery. Supportive treatment in diarrhoea of various causes. Disorders where a higher daily fibre intake is desirable eg, irritable bowel syndrome. Supportive treatment in mild to moderately raised cholesterol values in addition to dietary measures.
Dosage/Direction for Use
Adult & adolescents ≥12 yr Treatment of chronic constipation & in conditions in which easier bowel movements w/ soft stools are desirable 1 sachet bd-tds. Supportive treatment in diarrhoea, irritable bowel syndrome or increased cholesterol values 1 sachet bd to 6 times daily.
Administration
Should be taken with food: Stir the required amount into a glass of cold water (at least 150 mL) & drink immediately. Do not take when lying down or immediately before going to bed.
Contraindications
Hypersensitivity. Following any sudden change in bowel habits lasting longer than 2 wk. If no stool had passed after laxative administration. Blood in stool of unknown cause. Dysphagia or any other throat or pharyngeal problems. Abnormal narrowing of the GI tract. Disease of the gullet or stomach entrance. In cases of potential or existing ileus or megacolon syndrome.
Special Precautions
Prolonged constipation & irregular bowel movements lasting longer than 3 days or diarrhoea which lasts longer than 2 days or is accompanied by blood or raised temp; constipation w/ unclear abdominal symptoms, abdominal pain, nausea & vomiting; chest pain or vomiting, swallowing or breathing difficulties. Medical supervision is required if administered as an adjuvant to diet in hypercholesterolaemia. Known intolerance to certain sugars. Patients on a controlled Na diet. Childn <12 yr. Debilitated patients & the elderly.
Adverse Reactions
Flatulence, feeling of fullness, hard stools; allergic reactions.
Drug Interactions
Delayed intestinal absorption of other medicines taken concurrently eg, minerals (Ca, Fe, lithium, Zn), vit (vit B12), cardiac glycosides, coumarins, carbamazepine. Weakened effects of thyroid hormone. Intestinal obstruction may occur w/ medicines that inhibit the natural bowel movement (eg, loperamide, opium tinct). Adjustment of anti-diabetic therapy may be necessary.
