Dosage/Direction for Use
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to Unisia or dihydropyridines. Patients w/ diabetes on aliskiren fumarate except w/ significantly uncontrolled BP despite treatment w/ other antihypertensive therapy. Pregnancy.
Special Precautions
Hypersensitivity. Not indicated as primary drug for treatment of HTN. Patients w/ bilateral or unilateral renal artery stenosis; hyperkalemia. Closely observe patients on hemodialysis, on strict dietary salt restriction, under diuretic therapy, w/ hyponatremia, heart failure. May impair ability to drive & use machines. Renal/hepatic dysfunction. Elderly.
Adverse Reactions
Angioedema; shock, syncope or unconsciousness; acute renal failure; hyperkalemia; hepatic dysfunction or jaundice; agranulocytosis, decreased WBC & platelet count; rhabdomyolysis; interstitial pneumonia; hypoglycemia; AV block.
Drug Interactions
May intensify hypotensive effect w/ ACE inhibitors; β-blockers, nitroglycerin, sildenafil. Elevation of serum K level w/ K-sparing diuretics eg, spirinolactone, triamtelene, eplerenone, K-supplements. Enhanced antihypertensive effect w/ diuretics eg, furosemide, trichloromethiazide. Enhanced blockade of renin-angiotensin system w/ aliskiren fumarate. Increased renal tubule reabsorption w/ lithium carbonate. NSAIDs may decrease hypotensive effect. Amlodipine metabolism may be competitively inhibited w/ CYP3A4 inhibitors eg, erythromycin, diltiazem, ritonavir, itraconazole. Enhanced amlodipine metabolism w/ CYP3A4 inducers eg, rifampicin. May increase blood conc of amlodipine w/ grapefruit juice. Increased simvastatin AUC.
