Concise Prescribing Info
Per mL Ipratropium Br 250 mcg, fenoterol HBr 500 mcg
Bronchodilator for the prevention & treatment of symptoms in chronic obstructive airway disorders w/ reversible airflow limitation eg, bronchial asthma & especially chronic bronchitis w/ or w/o emphysema, consider concomitant anti-inflammatory therapy in patients w/ bronchial asthma & steroid-responsive COPD.
Dosage/Direction for Use
Adult including elderly & adolescent >12 yr Acute bronchospasm 1-2.5 mL (20-50 drops). Severe cases 4 mL (80 drops). Acute asthma Childn 6-12 yr 0.5-2 mL (10-40 drops), <6 yr ,<22 kg 0.1 mL/kg (2 drops). Max: 0.5 mL (10 drops).
Hypersensitivity to fenoterol HBr or atropine-like substances. Hypertrophic obstructive cardiomyopathy & tachyarrhythmia.
Special Precautions
Hypersensitivity. Discontinue if paradoxical bronchospasm occurs. Patients predisposed to narrow-angle glaucoma; insufficiently controlled DM, recent MI, severe organic heart or vascular disorders, hyperthyroidism, phaeochromocytoma, preexisting urinary tract obstruction eg, prostatic hyperplasia or bladder-neck obstruction; underlying severe heart disease eg, ischaemic heart disease, arrhythmia or severe heart failure; hypokalaemia; cystic fibrosis; acute, rapidly worsening dyspnoea; doping. May cause wheezing & breathing difficulties due to benzalkonium Cl. Prolonged use. Avoid contact w/ eyes. Concomitant use w/ other sympathomimetic bronchodilators. May affect ability to drive & use machines. Pregnancy & lactation.
Adverse Reactions
Cough, dry mouth, headache, tremor, pharyngitis, nausea, dizziness, dysphonia, tachycardia, palpitations, vomiting, increased heart rate & systolic BP, nervousness.
Drug Interactions
Not recommended in co-administration w/ other anticholinergic drugs. Enhanced bronchodilatory effect w/ other β-adrenergics, anticholinergics, xanthine derivatives (eg, theophylline). Reduced bronchodilation w/ β-blockers. Increased β2-agonist-induced hypokalaemia w/ xanthine derivatives, corticosteroids, diuretics. Increased susceptibility to arrhythmias w/ digoxin. Enhanced action w/ MAOIs, TCAs. Increased susceptibility on CV effects w/ halogenated hydrocarbon anaesth inhalation eg, halothane, trichloroethylene & enflurane.
