Resp tract, eye, kidney, urinary tract, genital, GIT, biliary tract, skin & soft tissue, bone & joint infections, otitis media, sinusitis, sepsis, intestinal decontamination in immunosuppressed patients.
Dosage/Direction for Use
Resp tract infection 200-400 bd. Acute, uncomplicated UTI 100 mg bd. Cystitis in premenopausal women 100 mg as single dose. Complicated UTI 200 mg bd. Extragenital gonorrhea 100 mg bd. Acute, uncomplicated gonorrhea 100 mg as single dose. Diarrhea 200 mg bd. Other infections 200-400 mg bd. Severe life-threatening infections 400 mg tds. Duration of treatment: Acute uncomplicated gonorrhea & cystitis 1 day. Kidney infections, UTI & abnormal cavity Up to 7 days. Osteomyelitis Max 2 mth. Other infections 7-14 days. Streptococcal & Chlamydia infections At least 10 days. CrCl 31-60 mL/min Max daily dose: 800 mg IV, ≤30 mL/min Max daily dose: 400 mg IV.
Hypersensitivity to quinolones. Pregnancy & lactation. Childn & adolescents.
Special Precautions
Epilepsy; history of CNS disorders; pseudomembranous colitis; CHF. Ensure adequate hydration. G6PD patients. Avoid excessive sun or UV light exposure. Renal fail­ure, nephrotic syndrome. Elderly.
Adverse Reactions
Nausea, diarrhea; rash. Moniliasis; eosinophilia, leukopenia; anorexia; headache, dizziness, insomnia, agitation, confusion, taste perversion; vomiting, dyspepsia, flatulence, abdominal pain; pruritis, maculopapular rash, urticaria; arthralgia; increased SGOT, SGPT, alkaline phosphatase, creatinine & BUN, abnormal liver function test.
Drug Interactions
Increased theophylline & methotrexate plasma levels; serum conc of clozapine, tacrine, ropinirole, tizanidine, duloxetine. Altered phenytoin levels. Prolonged bleeding time w/ anticoagulants. May lead to convulsions w/ NSAIDs. Increased serum creatinine by cyclosporin. Potentiated glibenclamide effect. Reduced renal clearance by probenecid.
