Repaglinide
Indications/Uses
Monotherapy or combination w/ metformin as adjunct to diet & exercise in adults w/ type 2 DM whose hyperglycemia cannot be controlled by diet, wt reduction & exercise.
Dosage/Direction for Use
Initially 0.5 mg titrated on wkly or bi-wkly basis. Patient transferred from another oral hypoglycaemic agent Initially 1 mg. Recommended max single dose: 4 mg, not to exceed 16 mg.
Administration
Should be taken with food: Take w/in 30 min before or 15 min after a meal. Patients who skip a meal (or add an extra meal) should be instructed to skip (or add) a dose for that meal.
Contraindications
Hypersensitivity. Type 1 DM, C-peptide -ve, diabetic ketoacidosis w/ or w/o coma. Concomitant use w/ gemfibrozil. Severe hepatic dysfunction.
Special Precautions
Discontinue use if exposed to stress eg, fever, trauma, infection or surgery. Increased risk of hypoglycaemia. Increased incidence of acute coronary syndrome eg, MI. Debilitated or malnourished patients. Combination w/ insulin secretagogues. May affect ability to drive & use machines. Hepatic & renal impairment. Pregnancy & lactation. Childn <18 yr. Elderly >75 yr.
Adverse Reactions
Hypoglycaemia; abdominal pain, diarrhoea.
Drug Interactions
CYP2C8 & 3A4 inhibitors. Increased plasma conc w/ ciclosporin. Enhanced &/or prolonged hypoglycaemic effect w/ gemfibrozil, trimethoprim, rifampicin, ketoconazole, itraconazole, clarithromycin, ciclosporin, deferasirox, clopidogrel, other antidiabetics, MAOIs, non-selective β-blockers, ACE inhibitors, salicylates, NSAIDs, octreotide, alcohol, anabolic steroids. Reduced hypoglycaemic effect w/ OCs, rifampicin, barbiturates, carbamazepine, thiazides, corticosteroids, danazol, thyroid hormones, sympathomimetics.
