Tamoxifen citrate
Dosage/Direction for Use
May be taken with or without food.
Hypersensitivity. Concurrent anastrozole therapy. Pregnancy.
Special Precautions
Increased incidence of endometrial changes. Patients who report abnormal gynaecological symptoms especially vag bleeding or presents w/ menstrual irregularities, vag discharge & symptoms eg, pelvic pain or pressure. Screen for thrombophilic factors for patients w/ prothrombotic risk. Long-term anticoagulant prophylaxis for patients who have multiple risk factors for venous thromboembolism (VTE). Discontinue immediately if VTE occurs. Risk of microvascular flap complications in delayed microsurgical breast reconstruction. Concomitant w/ potent CYP2D6 inhibitors (eg, paroxetine, fluoxetine, quinidine, cinacalcet or bupropion). Galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. May affect ability to drive or operate machinery. Women should be advised not to become pregnant & use barrier or other non-hormonal contraceptives. Premenopausal patients must be carefully examined to exclude pregnancy. Lactation. Childn.
Adverse Reactions
Fluid retention; hot flushes; nausea; vag bleeding & discharge; skin rash; fatigue. Uterine fibroids; anaemia; hypersensitivity; ischaemic cerebrovascular events, headache, light headedness, sensory disturbances (eg, paraesthesia & dysgeusia); cataracts, retinopathy; vomiting, diarrhoea, constipation; changes in liver enzymes, fatty liver; alopecia, leg cramp, myalgia; pruritus vulvae, endometrial changes (eg, hyperplasia & polyps); elevated triglycerides; thromboembolic events (eg, DVT, microvascular thrombosis & pulmonary embolism).
Drug Interactions
Increased effect of coumarin-type anticoagulants. Increased risk of thromboembolic events w/ cytotoxics. May reduce levels w/ rifampicin. Reduce efficacy in concomitant use w/ SSRI antidepressants (eg, paroxetine). Reduce plasma levels of CYP2D6 inhibitord (ie, endoxifen). Reduced efficacy w/ SSRI antidepressants (eg, paroxetine) & potent CYP2D6 inhibitors (eg, paroxetine, fluoxetine, quinidine, cinacalcet or bupropion).
