Amoxicillin trihydrate
Indications/Uses
Resp tract, genito-urinary tract, skin & soft tissue, biliary tract infection; acute, uncomplicated gonorrhea.
Dosage/Direction for Use
Cap Adult 250-500 mg every 8 hr. Max: 2-3 g/day. Ear, nose, throat, genitourinary tract &/or skin/skin structure infections Mild to moderate: 500 mg every 12 hr or 250 mg every 8 hr. Severe: 500 mg every 8 hr. Lower resp tract infections 500 mg every 8 hr. Gonorrhea (acute, uncomplicated, anogenital & urethral infections in males & females) 3 g as a single dose. Renal impairment CrCl 10-30 mL/min 250-500 mg every 12 hr, CrCl <10 mL/min 250-500 mg every 24 hr. Moderately dialyzable (20-50%) by hemo- or peritoneal dialysis Approx 50 mg/L of filtrate is removed by continuous arteriovenous or venovenous hemofiltration. Childn 7-12 yr 250 mg every 8 hr. Syr 20-40 mg/kg in divided doses every 8 hr.
Administration
May be taken with or without food: May be taken w/ meals for better absorption & to reduce GI discomfort.
Contraindications
Hypersensitivity to amoxicillin or penicillins.
Special Precautions
Patients w/ renal impairment, infectious mononucleosis. Cross-allergy w/ other β-lactams & cephalosporins.
Adverse Reactions
Hyperactivity, agitation, anxiety, insomnia, confusion, convulsions, behavioral changes, dizziness; acute exathematous pustulosis, erythematous maculopapular rash, erythema multiforme, Stevens-Johnson syndrome, exfoliative dermatitis, toxic epidermal necrolysis, hypersensitivity vasculitis, urticaria; nausea, vomiting, diarrhea, hemorrhagic colitis, pseudomembranous colitis, tooth discoloration; anemia, hemolytic anemia, thrombocytopenia, thrombocytopenia purpura, eosinophilia, leucopenia, agranulocytosis; elevated AST & ALT, cholestatic jaundice, hepatic cholestasis, acute cytolytic hepatitis; crystalluria.
Drug Interactions
Increased levels w/ disulfiram & probenecid. May increase effects of oral anticoagulants (warfarin). Potential additive effect w/ allopurinol. May increase exposure to methotrexate. Decreased effects w/ tetracyclines & chloramphenicol.
