Esomeprazole Mg
Dosage/Direction for Use
Tab Adult & adolescent ≥12 yr Treatment of erosive reflux esophagitis 40 mg once daily for 4 wk. An additional 4 wk for patients in whom esophagitis has not healed or have persistent symptoms. Long-term management w/ healed esophagitis to prevent relapse 20 mg once daily. Symptomatic treatment of GERD 20 mg once daily in patients w/o esophagitis. If symptom control has not been achieved after 4 wk, the patient should be further investigated. Adult In combination with an appropriate antibacterial therapeutic regimen for the eradication of H. pylori Healing of H. pylori associated duodenal ulcer or as prevention of relapse of peptic ulcers in patients with H. pylori associated ulcers: esomeprazole 20 mg w/ amoxicillin 1 g & clarithromycin 500 mg, all bd for 7 days. Patient requiring continued NSAID therapy Healing of gastric ulcers associated w/ NSAID therapy 20 mg once daily for 4-8 wk. Prevention of gastric & duodenal ulcers associated w/ NSAID therapy in patients at risk 20 mg once daily. Prolonged treatment after IV induced prevention of rebleeding of peptic ulcers 40 mg once daily for 4 wk after IV induced prevention of rebleeding of peptic ulcers. Treatment of Zollinger Ellison Syndrome 40 mg twice daily initially, 80-160 mg daily for maintenance, w/ doses >80 mg daily, doses should be divided & given bd. Granules (sachet) Treatment of endoscopically proven erosive reflux esophagitis Childn 1-11 yr w/ body wt of ≥10 kg to <20 kg 10 mg once daily for 8 wk. Childn 1-11 yr w/ body wt ≥20 kg 10 mg or 20 mg once daily for 8 wk. Symptomatic treatment of GERD 10 mg once daily for up to 8 wk.
May be taken with or without food: Tab: Swallow whole. For patients w/ swallowing difficulties, disperse tab in ½ glass of non-carbonated water. Do not use other liqd. Stir until tab dissolves into little pellets & drink immediately or w/in 30 min. Rinse glass w/ ½ glass of water & drink. Do not chew/crush tab/pellets. Mixt may be administered via a nasogastric tube. Granules: Add 1 sachet in 15-30 mL non-carbonated water. Stir until granules disperses & leave for a few min to thicken. Stir again & drink w/in 30 min. Do not chew/crush granules. Rinse w/ 15 mL water & drink. Mixt may be administered via a nasogastric tube.
Hypersensitivity to esomeprazole, substituted benzimidazoles or any other constituents of the formulation. Co-administration w/ nelfinavir.
Special Precautions
Exclude gastric malignancy before treatment. Patients on long-term & on-demand treatment. Patients w/ rare hereditary fructose intolerance, glucose-galactose malabsorption or sucrase-isomaltase insufficiency. Increased risk of gastrointestinal infections; co-administration w/ atazanavir; concomitant use w/ clopidogrel. Patients at risk of osteoporosis. Severe hypomagnesemia, especially for patients expected to be on prolonged treatment or take other medications that may cause hypomagnesemia. Severe renal & hepatic impairment. Pregnancy & lactation. Tab: Childn <12 yr. Granules: Childn <1 yr or <10 kg.
Adverse Reactions
Headache; abdominal pain, constipation, diarrhoea, flatulence, nausea/vomiting.
Drug Interactions
Decreased absorption of ketoconazole, itraconazole & erlotinib. Increased absorption of digoxin. Decreased serum levels of atazanavir & nelfinavir. Increased plasma conc of diazepam, citalopram, imipramine, clomipramine & phenytoin. Increased Cmax & AUC of cilostazol & cisapride. Elevated INR w/ warfarin & other coumarine derivatives. Increased exposure w/ clarithromycin. Decreased exposure to the active metabolite of clopidogrel. Decreased serum levels w/ CYP2C19 or CYP3A4 inducers (eg, rifampicin & St. John's wort).
