Contents
Per 50/500 mg Sitagliptin 50 mg, metformin HCl 500 mg. Per 50/1,000 mg Sitagliptin 50 mg, metformin HCl 1,000 mg
Indications/Uses
As adjunct to diet & exercise to improve glycemic control in type 2 DM inadequately controlled on metformin or sitagliptin alone, or patients already receiving combination sitagliptin & metformin.
Dosage/Direction for Use
Individualised dosage not exceeding max daily dose of sitagliptin 100 mg & metformin 2 g. Generally given twice daily. Patient inadequately controlled on metformin alone Usual starting dose: Sitagliptin 50 mg twice daily, plus the dose of metformin already being taken. Patient inadequately controlled on sitagliptin alone Usual starting dose: Sitagliptin 50 mg/metformin HCl 500 mg twice daily. Dose may be titrated up to sitagliptin 50 mg/metformin HCl 1,000 mg twice daily. Switching from co-administration of sitagliptin & metformin May be initiated at the dose of sitagliptin & metformin already being taken.
Administration
Should be taken with food.
Contraindications
Renal disease or renal dysfunction (serum creatinine levels ≥1.5 mg/dL for males & ≥1.4 mg/dL for females or abnormal CrCl) which may also result from conditions eg, CV collapse (shock); acute MI & septicemia. Acute or chronic metabolic acidosis, including diabetic ketoacidosis, w/ or w/o coma. Patients under radiologic studies involving intravascular administration of iodinated contrast materials. Type 1 diabetes.
Special Precautions
Hypersensitivity. Significant renal insufficiency including intrinsic renal disease & renal hypoperfusion, unstable or acute CHF/MI at risk of hypoperfusion & hypoxemia; dehydration, sepsis, impaired hepatic function. Monitor renal function regularly & vit B12 level annually. Avoid excessive alcohol intake, in concomitant w/ hypoglycemic agents, or drugs that may affect renal function, alter hemodynamics or interfere w/ metformin disposition, & intravascular contrast studies w/ iodinated materials. Pregnancy & lactation. Childn <18 yr. Elderly.
Adverse Reactions
Diarrhea, nausea, vomiting, flatulence, abdominal discomfort, indigestion, asthenia & headache.
Drug Interactions
Cationic drugs, digoxin, glyburide, furosemide, nifedipine; thiazides & other diuretics, corticosteroids, phenothiazines, thyroid products, estrogens, OCs, phenytoin, nicotinic acid, sympathomimetics, Ca-channel blockers, INH.
