Meropenem
Indications/Uses
Pneumonia & nosocomial pneumonias, urinary tract, intra-abdominal gynaecological eg, endometritis, skin & skin structure infections; meningitis, septicaemia. Empiric treatment for presumed infections in febrile neutropenia as monotherapy or in combination w/ antiviral or antifungal agents. Polymicrobial infections.
Dosage/Direction for Use
Adult Pneumonia, urinary tract, gynaecological infections eg, endometritis, skin & skin structure infections 500 mg IV 8 hrly. Nosocomial pneumonias, presumed infections in neutropenia, septicaemia 1 g IV 8 hrly. Meningitis 2 g 8 hrly. Renal impairment CrCl 26-50 mL/min 1 unit dose 12 hrly, 10-25 mL/min ½ unit dose 12 hrly, <10 mL/min ½ unit dose 24 hrly. Childn ≥3 mth 20-40 mg/kg 8 hrly. Max: 2 g 8 hrly. Intra-abdominal infection 20 mg/kg 8 hrly. Meningitis 40 mg/kg 8 hrly; >50 kg Intra-abdominal infection 1 g 8 hrly. Meningitis 2 g 8 hrly. Infusion rate: 15-30 min IV or 5-20 mL IV bolus over 3-5 min .
Contraindications
Hypersensitivity. MRSA infections.
Special Precautions
Hypersensitivity reaction to β-lactam antibiotics. Monitor transaminase & bilirubin levels in patients w/ hepatic disease. Consider diagnosis of pseudomembranous colitis in patients who develop diarrhea. Co-administration w/ potentially nephrotoxic drugs. May affect ability to drive or operate machinery. Pregnancy & lactation. Childn & infant <3 mth.
Adverse Reactions
Inflammation, thrombophlebitis, pain at inj site, angioedema, anaphylaxis, rash, pruritus, urticaria, abdominal pain, nausea, vomiting, diarrhea, pseudomembranous colitis, reversible thrombocythaemia, eosinophilia, thrombocytopenia, leucopenia, neutropenia, increases in serum conc of bilirubin, transaminases, alkaline phosphatase & lactic dehydrogenase, headache, paraesthesiae, convulsion, oral & vag candidiasis.
