Bisoprolol fumarate
Indications/Uses
Stable moderate to severe chronic heart failure w/ declined systolic ventricular function (ejection fraction 35% based on echocardiogram) adjunctive to ACE inhibitor, diuretics, & glycosides. 5 mg: HTN & coronary artery disease (angina pectoris).
Dosage/Direction for Use
HTN & angina pectoris 1 tab daily in the morning. Mild symptoms: 5 mg daily. Can be increased to 20 mg daily, if needed. Patients w/ stable-chronic heart failure 1.25 mg once daily for 1 wk, if well-tolerated, doses can be increased to 2.5 mg once daily for the next wk, then can be further increased to 3.75 mg once daily for the next wk. If well tolerated, can be increased to 5 mg once daily for 4 wk, then 7.5 mg once daily for 4 wk, then 10 mg once daily for maintenance therapy. Max: 10 mg once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Acute heart failure or decompensated heart failure requiring inotropic IV, cardiogenic shock, 2nd or 3rd degree AV block w/o pacemaker, sick sinus syndrome, SA block, symptomatic bradycardia (<60 times/min before therapy), symptomatic hypotension (systolic pressure <100 mmHg), severe bronchial asthma or COPD, peripheral artery disease & Raynaud's syndrome, untreated pheochromocytoma, metabolic acidosis. Pregnancy & lactation.
Special Precautions
HTN, angina pectoris w/ heart failure, psoriasis, pheochromocytoma, bronchospasm, DM w/ fluctuating blood sugar, strict fasting, desensitization therapy, 1st degree AV block, Prinzmetal's angina, peripheral artery disease.
Adverse Reactions
Bradycardia, dizziness, headache, cold sensation & numbness, hypotension, nausea, vomiting, diarrhea, constipation, fatigue, malaise.
Drug Interactions
Antiarrhythmia class I (disopyramide, quinidine, lidocaine, phenytoin, flecainide, propafenone), Ca antagonist, centrally-acting antihypertensive (clonidine, methyldopa, moxonodine, rilmenidine), amiodarone, tacrine, insulin & oral antidiabetes, anesth drugs, digitalis, OAINS, isoprenaline, dobutamine, noradrenaline, adrenaline, TCAs, barbiturate, phenothiazines, mefloquine, ergotamine derivate, rifampicin, MAOI.
