Methylprednisolone

Indications/Uses

Collagen, allergic & autoimmune, dermatologic, ophth, GI, endocrine, rheumatic & haematologic disorders, malignancy.

Dosage/Direction for Use

Initially 4-48 mg daily. Multiple sclerosis Larger initial dose (≥100 mg) is used for severe condition (160 mg daily for 7 days followed by 64 mg alternately for 1 mth). Long term large doses (1-2 wk) needs to be tapered off. Alternate day therapy for patients who need therapy >2 wk to minimize adverse effects: Doses for 2 days are given one time in the morning alternately eg, day 1, day 3, day 5.

Administration

Should be taken with food.

Contraindications

Hypersensitivity. TB, systemic fungal infection, varicella, herpes simplex keratitis, peptic ulcer, Cushing's syndrome, acute psychosis, recent immunization.

Special Precautions

Long term large doses (1-2 wk) need to be tapered off. Vaccination during treatment. Patients w/ history of repeat systemic corticosteroid therapy, especially if given ≥3 wk 32 mg daily. TB. Pregnancy.

Adverse Reactions

Dyspepsia, peptic ulcer w/ tendency of perforation & bleeding, risk of opportunistic infection, muscle weakness, osteoporosis, fluid & electrolyte disturbances, impaired wound healing, striae, petechiae & ecchymosis, irregular menstruation, growth suppression in childn, mood swings, depression, increased intracranial pressure, increased IOP, glaucoma, papilledema, cataracts, leucocytosis, hypersensitivity, thromboembolism, w/drawal symptoms (acute adrenal insufficiency, hypotension, even death).

Drug Interactions

Cardiac glycosides, diuretics, anticoagulants, antidiabetic agents, rifampicin, phenytoin, barbiturates, NSAIDs, erythromycin, ketoconazole.
