Indications/Uses
Acute bacterial exacerbation of chronic bronchitis, community-acquired pneumonia, complicated UTI & acute pyelonephritis. FC caplet: Acute maxillary sinusitis, uncomplicated skin & skin structure infections. Infusion: Acute bacterial sinusitis, complicated skin & skin structure infections, nosocomial pneumonia, chronic bacterial prostatitis.
Dosage/Direction for Use
Usual dose: 250-500 mg once daily for 7-14 days orally or by slow infusion over 60 min every 24 hr or 750 mg by slow infusion over 90 min every 24 hr. Acute bacterial exacerbation of chronic bronchitis 500 mg daily orally or by infusion for 7 days or 750 mg by infusion for 3-5 days. Community-acquired pneumonia 500 mg daily orally or by infusion for 7-14 days or 750 mg by infusion for 5 days. Acute maxillary sinusitis 500 mg daily orally for 10-14 days. Acute bacterial sinusitis 500 mg by infusion for 7-14 days or 750 mg for 5 days. Uncomplicated skin & skin structure infections 500 mg daily for 7-10 days. Nosocomial pneumonia 750 mg by infusion for 7-14 days. Complicated UTI & acute pyelonephritis 250 mg daily for 10 days orally or 250-750 mg by infusion once daily depends on the type & severity of the presumed causative pathogen usually for 7-14 days. Chronic bacterial prostatitis 500 mg by infusion for 28 days. Acute bacterial exacerbation of chronic bronchitis, community-acquired pneumonia, acute maxillary sinusitis, uncomplicated skin & skin structure infections Patient w/ CrCl >50 mL/min Initially 250 mg by infusion 24 hrly followed by 500 mg 24 hrly, CrCl 20-50 mL/min 250 mg by infusion 24 hrly, CrCl 20-49 mL/min Initially 500 mg orally followed by 250 mg 24 hrly, CrCl 10-19 mL/min Initially 500 mg followed by 250 mg 48 hrly. Hemodialysis & chronic ambulatory peritoneal dialysis patient Initially 500 mg orally followed by 250 mg 48 hrly. Complicated UTI & acute pyelonephritis Patient w/ CrCl 10-19 mL/min Initially 250 mg followed by 250 mg 48 hrly.
Administration
May be taken with or without food: Ensure adequate fluid intake.
Contraindications
Hypersensitivity. Epilepsy, history of tendon disorders related to fluoroquinolone administration. Pregnancy & lactation. Childn & adolescent <18 yr.
Special Precautions
Avoid excessive exposure UV light or sunlight. Known or suspected CNS disorders, renal insufficiency; history of convulsion. Infusion: Avoid physical activities & other activities involving affected tendon. Discontinue use if patient is diabetic. May impair ability to drive or operate machinery. Elderly.
Adverse Reactions
Agitation, confusion, hallucinations, tremors, drowsiness, dizziness, headache, sleep disturbances, skin rash, itching or redness, tendinitis or tendon rupture, abdominal or stomach pain/discomfort, constipation, diarrhea (may be bloody), nausea, vomiting, taste perversion, fever, allergic pneumonitis, anaphylaxis, encephalopathy, Stevens-Johnson syndrome. Infusion: Vag candidiasis, itching, discharge, pseudomembranous colitis.
Drug Interactions
Oral hypoglycemic agents or insulin, NSAIDs, warfarin, theophylline. FC caplet: Antacids containing Al or Mg; Fe sulfate, sucralfate, Zn.
