Indications/Uses
Inflammatory & degenerative forms of rheumatism: RA, juvenile RA, ankylosing spondylitis, OA & spondylarthritis, painful syndromes of the vertebral column, non-articular rheumatism; acute attacks of gout; post-traumatic & post-op pain, inflammation & swelling; primary dysmenorrhoea.
Dosage/Direction for Use
SR tab Initial daily dose 100 mg. Adult Initial daily dose 100-150 mg. Milder cases & for long-term therapy: 75-100 mg daily. Nocturnal pain & morning stiffness Up to max daily dose: 150 mg can be supplemented by supp at bedtime. Primary dysmenorrhea 50-150 mg, may be increased up to max: 200 mg daily. Adolescent & childn >1 yr 0.5-2 mg/kg daily in 2-3 divided dose. Juvenile RA Max dose: 3 mg/kg daily in divided doses. Supp Childn <14 yr 12.5 mg.
Administration
Should be taken with food: SR tab: Swallow whole, do not divide/chew.
Contraindications
Hypersensitivity to aspirin or diclofenac. Advanced blood abnormality, peptic ulcer, severe cardiac failure. Advanced hepatic & renal impairment.
Special Precautions
Discontinue use if rash occurs; monitor for signs of hypersensitivity. Not recommended in patients w/ established CV disease (CHF, established ischemic heart disease, peripheral arterial disease) or uncontrolled HTN; risk factors for CV disease eg, HTN, hyperlipidaemia, DM, smoking. Risk of GI ulceration, bleeding & perforation; peptic ulcer; asthma, bronchospasm. Monitor blood counts, renal & hepatic function. High-dose & long-term use. Concomitant use w/ corticosteroids. May affect ability to drive & use machines. Severe hepatic or renal damage. Women of childbearing potential. Not recommended during 3rd trimester of pregnancy & lactation. Elderly or debilitated patients.
Adverse Reactions
Epigastric pain, eructation; slight dizziness or headache; anaphylactoid reactions, mild skin rash, peripheral oedema; depression; drowsiness; insomnia, blurred vision; hepatic & renal impairment; agranulocytosis, thrombocytopenia.
Drug Interactions
Increased plasma conc of lithium, methotrexate, warfarin. Reduced effects of diuretics, β-blockers. Altered plasma conc of sulfonylureas, captopril, triamterene.
