Losartan K
Indications/Uses
HTN. Reduce risk of stroke in patients w/ HTN & left ventricular hypertrophy. Diabetic nephropathy w/ elevated serum creatinine & proteinuria in patients w/ type 2 diabetes & history of HTN.
Dosage/Direction for Use
HTN 50 mg once daily, may be increased to 100 mg as single or 2 divided doses. Intravascular vol-depletion eg, patient treated w/ high-dose diuretics Initially 25 mg once daily. HTN w/ left ventricular hypertrophy Initially 50 mg once daily w/ low-dose hydrochlorothiazide, may be increased to 100 mg once daily based on BP response. Renal protection in type 2 diabetic patient w/ proteinuria & HTN Initially 50 mg once daily, may be increased to 100 mg once daily based on BP response.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Hyperkalaemia; vol-depletion (eg, those treated w/ high-dose diuretics), hypotension; renal artery stenosis. Monitor serum K conc. Correct vol depletion prior to therapy. Avoid concomitant use w/ K-sparing diuretics. Hepatic & renal impairment. Not to be used during pregnancy. Lactation. Elderly.
Adverse Reactions
Dizziness, headache, dose-related orthostatic hypotension.
Drug Interactions
Decreased conc w/ rifampin. Decreased active metabolite & increased conc w/ fluconazole. Increased serum K w/ K-sparing diuretics (eg, spironolactone, triamterene, amiloride), K supplements, K-containing salt substitutes. Blunted antihypertensive effect w/ indomethacin.
