Contents
Natural conjugated estrogens
Indications/Uses
Moderate to severe vasomotor symptoms associated w/ estrogen deficiency. Prevention & management of osteoporosis associated w/ estrogen deficiency. Atrophic vaginitis & urethritis. Female hypoestrogenism.
Dosage/Direction for Use
Vasomotor symptoms, atrophic vaginitis & urethritis associated w/ estrogen deficiency 0.3-1.25 mg daily. Osteoporosis Initially 0.3 mg daily. Female hypoestrogenism 0.3-1.25 mg daily administered cyclically eg, 3 wk on & 1 wk off. Adjust dosage depending on severity of symptoms & endometrial responsiveness.
Administration
May be taken with or without food: Swallow whole, do not divide/chew/crush or dissolve tab in mouth.
Contraindications
Hypersensitivity. Known or suspected estrogen-dependent neoplasia & breast cancer, thrombophilic disorders. Undiagnosed abnormal uterine bleeding. Active or history of confirmed arterial thromboembolic or venous thromboembolic disorders. Active or chronic hepatic dysfunction or disease. Known or suspected pregnancy.
Special Precautions
Increased risk of MI, pulmonary embolism, invasive breast & ovarian cancer. CV risk, stroke, venous thromboembolism; endometrial, breast & ovarian cancer; dementia, gallbladder disease, hypercalcemia, palliative therapy in men, angioedema, fluid retention, hypertriglyceridemia, elevated BP, exacerbation of asthma, epilepsy, migraine w/ or w/o aura, otosclerosis, porphyria, SLE, hepatic hemangiomas, endometriosis; hypocalcemia, hypothyroidism; impaired glucose tolerance. History of cholestatic jaundice. Hepatic impairment. Not to be used during pregnancy & lactation. Childn. Elderly >65 yr.
Adverse Reactions
Alopecia; arthralgia, leg cramps; abnormal uterine bleeding, breast pain, tenderness, enlargement & discharge, leukorrhea; increased/decreased wt, increased triglycerides.
Drug Interactions
Reduced plasma conc w/ CYP3A4 inducers eg, St. John's wort, phenobarb, phenytoin, carbamazepine, rifampicin & dexamethasone. Increased plasma conc w/ CYP3A4 inhibitors eg, cimetidine, erythromycin, clarithromycin, ketoconazole, itraconazole, ritonavir & grapefruit juice. Reduced response to metyrapone.
