Bisoprolol fumarate
Indications/Uses
Management of HTN & angina pectoris.
Dosage/Direction for Use
HTN/angina pectoris Initially 5 mg once daily, may be increased to 10 mg once daily. Max: 20 mg once daily. Stable chronic heart failure 1.25 mg once daily for 1 wk, if well tolerated increase dose to 2.5 mg once daily for a further wk, then increase to 3.75 mg once daily for a further wk, then increase to 5 mg once daily for the following 4 wk, then increase to 7.5 mg once daily for the following 4 wk, then increase to 10 mg once daily for maintenance treatment. Max: 10 mg once daily.
Administration
May be taken with or without food.
Contraindications
Untreated cardiac failure, cardiogenic shock, SA block, 2nd/3rd degree AV block, marked bradycardia, severe asthma, sick sinus syndrome, hypotension, severe peripheral circulatory disturbances, untreated tumours of the adrenal gland (phaeochromocytoma), metabolic acidosis.
Special Precautions
Do not stop treatment abruptly. DM w/ extremely fluctuating blood glucose levels, strict fasting, ongoing desensitisation therapy, 1st degree AV block, Prinzmetal's angina, peripheral arterial occlusive disease, history of psoriasis, asthma or chronic obstructive airway disease; thyrotoxicosis, general anesth. Patients w/ prolonged PR conduction interval, poor cardiac reserve & Raynaud's syndrome. Severe renal & hepatic impairment. Pregnancy & lactation.
Adverse Reactions
Bradycardia, worsening of heart failure, dizziness, headache, GI disturbances, coldness or numbness in the extremities, hypotension, asthenia, fatigue.
Drug Interactions
Class I & III antiarrhythmics; verapamil, nifedipine; centrally-acting antihypertensives, topical β-blockers, parasympathomimetics, insulin or oral antidiabetics, anesth, cardiac glycosides, NSAIDs, β-sympathomimetics; sympathomimetics that activate both β- & α-adrenoceptors; TCAs, barbiturates, phenothiazines; mefloquine, MAOIs.
